Investigation Summary
On August 16, 1998, Employee #1 sustained a dislocation of the left ankle when it became caught between two pallet jacks. Employee #1 was operating a stand-up pallet jack, a type of motorized hand truck, and was in the process of parking the pallet jack in a designated parking area in preparation for taking a scheduled break. His left foot became caught between two pallet jacks and he was hospitalized for a dislocation of the left ankle.
