Employee's foot was run over by backing forklift

At approximately 5:00 p.m. on September 8, 1998, Employee #1, a citrus picker, sustained multiple fractures to the left foot when it was run over by a backing forklift. Employee #1 was transported to JFK Hospital, Indio, California, where a re-constructive orthopedic surgery was performed. The forklift operator was not looking in the direction of travel. The alarm system was working properly. Employee #1 had his back turned to the forklift and was not paying attention to the forklift movement.
