Employee's arm caught in rollers

At 6:00 p.m. on August 31, 1998, Employee #1, a temporary seasonal packer, had been directed to get underneath a moving conveyor system and clean the rollers. She sustained multiple fractures to the left arm when the arm was caught in the rollers of a moving cookie conveyor system at a bakery. Employee #1 was transported to a medical center where orthopedic surgeons reconstructed the arm with plates, pins, and screws. It was not the bakery's policy to de-energize the conveyor system during cleaning operations, nor was Employee #1 given training or instructions on the hazards unique to the cleaning operation.
