Employee's hip fractured when loader rolls over on incline

Employee #1 was asked by his foreman to smooth out a road that was being pioneered on a level above the quarry. He was back dragging the blade of his CAT 998 loader on a steep incline when the vehicle's engine stalled. The loader began rolling backward down the roughed out road. The rear wheels struck the high side of the grade and the loader flipped over 1 1/2 times. Employee #1 was ejected from his seat and the ROPS was ripped off the cab. He was struck by the vehicle and suffered a fractured hip and a compound fractures of his leg. Employee #1 had years of experience on this equipment and was very well trained and qualified. He was not wearing his seat belt.
