Investigation Summary
At approximately 6:30 a.m. on November 5, 1997, Employee #1 was reaching through the mast assembly of a forklift to remove a length of wire tie wrap. He inadvertently activated the DOWN control and the forklift mast descended, crushing his left arm. Employee #1 was taken to Loma Linda University Medical Center, where he was hospitalized for five weeks.
