EMPLOYEE PINNED AGAINST RACK- FRACTURES VERTEBRAE

At approximately 2:20 a.m. on September 22, 1997, Employee #1, a warehouse worker, became pinned against a rack after the forklift he was operating hit the rack. He was hospitalized for five days and suffered fractures to his vertebrae (positions L-1, L-2, L- 3, and T-11).
