Employee's fingertip amputated in conveyor chain
At approximately 7:30 p.m. on March 31, 1997, Employee #1 was filing carbon ridges from steel slats on a slow-moving conveyor. His gloved left index finger became caught in the conveyor chain, amputating the distal joint.
