Investigation Summary
At approximately 3:00 a.m. on May 25, 2004, Employee #1 was sweeping shredded carrots near a cutter, Cutter Number 23. The employee observed some shredded carrots in the cutter and reached into the drive belt area and his fingers were caught between the drive belt and the sprocket. Employee #1 was hospitalized, where he was treated for an amputation.
