Employee Is Run over by Truck and Sustains Amputation

On March 30, 2006, Employee #1 was filling out paperwork in the pallet yard for his employer. The driver of the semi-truck which was parked approximately 3 ft away from Employee #1's parked forklift. The driver started the truck and depressed the clutch on the truck. The driver did not press the brake pedal and the truck rolled back pinning Employee #1's legs between the forklift and the truck trailer causing a serious injury to his legs. Employee #1 sustained a serious injury to his legs resulting in an amputation and was hospitalized.
