Investigation Summary

Investigation Nr: 170846398
Event: 06/20/1997
Employee injured in fall from conveyor belt

Employee #1 worked for a cement batching plant that also had a fleet of mobile delivery trucks. At the time of the accident, he was working as a dispatcher and operating the semi-automated plant. A screw conveyor feeding the batch plant jammed. Employee #1 walked up an inclined conveyor belt to the equipment platform but found the screw was in the wrong position to be worked on. He was walking back down the conveyor to the control house when he tripped on a conduit that crossed the conveyor at a 90 degree angle. Employee #1 lost his footing and fell approximately 10 to 12 ft, landing on the cement apron of the control house. He suffered a broken right wrist and dislocated left elbow. Employee #1 stated that on previous occasions he had used the conveyor belt to reach the elevated equipment platform.
