Employee Fractures Foot and Ankle When He Overturns Forklift

At approximately 12:15 a.m. on January 15, 2005, Employee #1 was operating a Crown RC-3000 forklift, RC044, Serial Number 1A225268, in the dock area of a warehouse building, Number ML3. He turned in the direction of a closed dock door and was unable to stop before he and the vehicle crashed through the roll-up door and fell 4 ft off the deck to the pavemen. He alleged the braking systems failed however, regular preventative maintenance, preusage inspections by the operator and post-accident evaluation indicate the brakes were operating properly and the vehicle had normal stopping power. The overhead guard of the forklift came to rest on his left foot, crushing it. He was hospitalized with a fractured foot and ankle.
