Employee crushes his finger in a X-ray machine

At approximately 6:45 p.m. on September 10, 2003, an employee, a design engineer, came to test the X-ray machine. He noticed a problem of noise on the gear box of the machine. To feel the vibration noise on the gear box, the employee placed his left hand and ear on the gear box and his right hand on the base plate of the gear box at which time the rotating crank arm moved past the base plate and pinched his right index and middle fingers against the edge of the base plate causing amputation of his right index finger and crushing injury of his right middle finger. He was hospitalized and treated for his injuries.
