Investigation Summary
At approximately 6:00 p.m. on July 23, 2003, Employee #1, a temporary employee, was operating a "Pines" Horizontal Rotary Bender power press in the manufacturing department. During normal operation of the press, it completes the bending cycle when the press is activated. At the time of the accident, the employee inadvertently hit the power control button with his right hand while positioning a 1.25-inch-diameter by 31.5-inch-long SKCA steel tube on the mandrel of the press machine with his left hand. The horizontally rotating holding die closed in to hold the steel tube and two fingers on his left hand were caught between the steel tube and the closing die. He was transported to Kaiser Permanente Hospital, where he was hospitalized for 3 days.
