Investigation Summary
At about 5:41 p.m. on October 1, 2004, Employee #1, a fork lift operator, was backing a fork lift (Raymond Model Number R35.C35QM, Serial Number R35-03-05357) in the receiving area of a distribution center towards a dock door. When the forklift moved too close to a metal dock ramp, he went to apply the emergency brakes. As he lifted his right foot from the foot control pedal, the forklift tilted against the trailer door. His left foot then became crushed between the forklift's frame and the latch of the trailer door. Employee #1 was hospitalized for an amputation of his left foot.
