Investigation Summary
At 3:35 p.m. on October 5, 2004, Employee #1, a supervisor, was removing jammed material from a fiber machine (Cotton Fluffing, Model Number 2376) while the machine was in manual mode. To do this, he accessed the chain & sprocket drives from a 2.75-in. gap opening (located 29 in. above the ground) between its cover and the machine's frame. When a coworker inadvertently moved the chains, Employee #1's right hand became caught in the chain drives. He was treated at Loma Linda University Medical Center for amputation of his index, middle, and ring fingers.
