Forklift Operator Amputates Fingers While Repairing Forklift

At approximately 1:00 p.m. on August 24, 2004, a forklift operator was inserting a detached fork of a Caterpillar DPL40 forklift, with his left hand. The employer manufactured and provided full service repair of pressure vessels and tanks. The employee worked in the southeast section of the manufacturing facility. When the employee placed the notched fork on the horizontal mast, his little and left ring fingers were caught between the notch and the mast. It caused partial amputation to these fingers. The employee was taken to Anchor Care Industrial Medical Clinic, in Pomona, California, for treatment. He was then transported to the Magnolia Surgical Center, in Colton, California, where outpatient surgery was performed. The injury did not require hospitalization.
