Investigation Summary
At about 1:15 p.m. on July 28, 2004, Employee #1, a process technician/machine operator with AMA Plastics Inc., was wipe-cleaning the molds of a Toyo injection molding machine (Model Number Ti-30BS) while it was in semi-automatic mode. He was standing on a worktable top 33 in. above the ground to access the top of the machine. Employee #1 lost his balance and his left knee inadvertently bumped the machine's front door, causing it to close. His left hand then became caught between the molds, and its lead pin pierced through Employee #1's palm. He was hospitalized at Riverside Community Hospital for treatment of the puncture and surgery.
