Employee injured while operating forklift

At approximately 1:30 a.m. on June 9, 2004, Employee #1 was operating a Raymond, Model Number EASR45TTMB, stand-in type forklift at a main distribution center engaged in receiving, storing and shipping of office equipment and supplies. As Employee #1 attempted to stow three pallets of merchandize on a rack, located in isle 49 of the replenish department, she panicked, lost control of the forklift and backed into the stationary racks, crushing her left arm. Employee #1 was initially taken to the Arrowhead Regional Medical Center in Colton, California and was later transferred to the Kaiser Permanente Hospital in Fontana, California, where she underwent surgery. Employee #1 was released four days later.
