Investigation Summary
On May 2, 2005, Employee #1, a production worker, was at the entrance of an oven in which wet cement rubber molds were cured. These molds were placed on pallets. Three rows of pallets were stored against the west wall of the curing chamber. Each row had pallets stacked three high and five deep. While Employee #1 was holding the western door of the chamber, a three-high stack of pallets with the wet cement molds (24 boxes each weighing 60 pounds) fell from the southwest row of pallets. He fell to the concrete floor under the mold boxes/trays. Employee #1 was transported to Arrowhead Regional Medical Center, where he died of his injuries.
