Employee Sustains Fractures in Fall

At about 12:45 p.m. on February 04, 2005, Employee #1, of Service Connection, Inc., in Ontario, CA, was inventorying merchandise at this facility. He was twelve and a half feet above the floor in a cage platform that was mounted on a forklift. A coworker, Employee #2, was the operator of the sit-in-type Mitsubishi model number FGC18K forklift, with serial number AR81D00753. As Employee #2 was lowering the cage platform, it got stuck on a wooden pallet that extended beyond the rack edge. The cage platform slipped off the forks, and the cage and Employee #1 fell to the concrete floor. The cage railing hit Employee #1 on his left leg, causing multiple fractures of his left leg and left knee. Employee #1 was taken to the Arrowhead Regional Medical Center in Colton, CA, for treatment. Surgery was performed the same day. Employee #1 was hospitalized for 6 days. Employee #1 was not a regular employee of Service Connection, which is primarily a warehousing facility for receiving and storing merchandise and then packaging and shipping orders for wholesale merchants.
