Temporary Employee's Toe Is Amputated by Falling Turntable

At about 3:00 p.m. on May 9, 2005, Employee #1 was working for a company that manufactured automotive hoses. He was a temporary employee provided by the employment service company Independent Staffing Solutions. Working in the inner line area, he and his group leader were removing a dish from a tripod turntable. The 50-in.-diameter dish had hose inner tubing wound on it; together the hose and the dish weighed approximately 126 pounds. The tripod turntable had a 30-in.-diameter steel disk; it weighed 78 pounds. Moving the dish, the two workers did not lift it high enough to detach it completely from the base of the turntable. The turntable tipped over, falling on Employee #1's right foot and crushing his big toe. Employee #1 was hospitalized, and the toe was amputated.
