Employee Fractures Hand When Caught in Drive Gear Shaft

At 6:02 p.m. on January 14, 2006, Employee #1, a stacker at a production plant, was assigned clean-up duties of the production line, near the facilities mixing system. Shortly before clean-up was to commence, Employee #1 proceeded to this area. While he was waiting for the equipment to stop, Employee #1 attempted to use his left hand to brush off sand and cement debris near an unguarded Boston gear shaft, Series 700, Category Number 7565B961, Ratio 5, of a mixing beater. While Employee #1 was removing the debris, his Towa Power Grab glove on his left hand became caught in the bolts used to secure the mixing beater to the mixer drive gear shaft. Employee #1 was taken to Loma Linda University Medical Center, where he was hospitalized and treated for a fractured left hand. The accident investigation revealed that normal safety procedures of the facility required that all production line equipment be stopped and lockout/tagout procedures be followed before clean-up can be conducted.
