Employee Dies after Fall from Shelf

At approximately 3:33 p.m. on January 16, 2006, Employee #1 slipped after climbing onto shelving, fell, and struck his head on a concrete floor. He died of head injuries on January 20, 2006. The employer did not require the use of a platform or ladder for access to the shelving.
