Investigation Summary
At approximately 11:15 a.m. on November 8, 2005, Employee #1, an equipment operator II, supervised by a supervisor, was assisting the California Department of Transportation's (Caltrans') tree crew to remove tree and brush debris. A dead tree was being removed in the city of Ramona on Route 67 at State Highway 78. Employee #1 was feeding limbs into the brush chipper, Vermeer 1250 Brush Chipper, on and off for approximately 2.5 hours. Employee #1 pushed a limb, 6 in. in diameter and 1-ft to 2-ft long into the chipper's feed rollers and the chipper kicked it back out. Employee #1 then grabbed the same limb a second time and pushed it again into the feed rollers. At that time, he kept his hand attached to the top of the limb. The chipper kicked the same limb back a second time, slamming the top of Employee #1's hand against the upper part of the metal enclosure in front of the feed rollers and behind the feed table. This sharp blow to Employee #1's left hand resulted in a compound fracture of his left index finger. Employee #1 was transported to Palomar Medical Center in Escondido, California. He was hospitalized.
