Employee Falls from Scissor Lift and Later Dies

At approximately 4:30 p.m. on February 24, 2007, Employee #1 was working on a scissor lift at a height of approximately 15 ft while traveling on a surface with on even surface. The scissor lift was a 1995 Model Number SM2232E, Serial Number 33426, with a maximum platform 16 ft, Load rating 500 pounds, maximum occupancy 2, maximum hydraulic pressure 3,000 psi, and maximum travel height 16 ft. The scissor lift had a lot of sway while it moved. According to witness the scissor lift shook violently from side to side then it fell over. As it fell, Employee #1tried to hold on and hit head first on pavement. Employee #1 suffered major head trauma and consequently died on the way to the hospital.
