Investigation Summary

Investigation Nr: 201070141
Event: 09/09/2006
Employee Is Killed in Fall from Scissor Lift

An electrician was working from a rented Genie scissor lift (Model No. GS-2632) during the construction of a department store. He was working approximately 4.3 meters above the ground, pulling fire alarm system wiring through a nearby wall-mounted box. The wiring originated across the room. A supervisor in another elevated Genie scissor lift was feeding the wiring to the electrician. When the wiring stuck, the electrician stood and moved toward the front of the scissor lift to get close to the box. He fell off the platform, landing on the ground. The electrician died of injuries sustained in the fall. He had not received training on the safe use of elevated platforms before using the scissor lift, and he was not wearing any personal fall protection equipment.
