Investigation Summary
At approximately 2:30 p.m. on April 2, 2004, Employee #1, a 52 year-old teaching assistant, was using a ladder to climb onto the roof of an elementary school in preparation for an egg drop experiment. The experiment involved dropping student designed containers with raw eggs from the roof of the school building. The ladder used to access the roof was a Werner Type III, 20-foot aluminum extension ladder that was placed at a height of 11 feet. The ladder was set up on top of a tarp, which was laid out over a blacktop to contain the egg debris. The ladder was not tied off or secured to the roof. Additionally, it was damp outside due to a light rain earlier that afternoon. A coworker climbed the ladder to the roof and waited for Employee #1 to follow. As Employee #1 ascended the ladder, reached the height of the roof, and was about to step onto the roof, the ladder slipped beneath her. She fell to the ground, landing on her back. No one was holding the ladder from above or below. Employee #1 was transported to Kaiser Hospital where she was hospitalized and treated for a broken vertebra, bruises, and a cut on her shin. She was released five days later.
