Investigation Summary
At approximately 10:30 a.m., on December 5, 2012, Employee #1 of All Seasons Painting & Maintenance used a 24-foot extension ladder to paint the gutter at the eaves of the 21 feet high second story roof. Employee #1 had been employed with the company for two days when the accident occurred. Prior to the accident, the company owner observed Employee #1 working from the ladder for 15 minutes; then the company owner went to other buildings. The ladder was not tied, secured or blocked to prevent it from displacement. According to the property owner, Employee #1 went up and down the ladder two times before the accident happened. The property owner heard the noise, came outside, and saw Employee #1 was on the ground with the ladder laid over him. The property owner called other near-by workers and the company owner. There were no actual witnesses to the accident. Paramedics were called immediately. Employee #1 was transported to the Long Beach Memorial Hospital. Employee #1 worker suffered serious head injured and died one week later at the hospital.
