Accident Investigation Summary
Summary Nr: 201069101Event: 07/25/2005Employee Is Killed From Asphyxia
At approximately 4:00 p.m. on July 25, 2005, Employee #1, a storage yard manager for a wholesale racking system supplier, was tying metal shelving units together that were stored on wood posts on the ground in an upright position. Each rack measured 38-feet long by 4-feet wide and weighed 270 lbs. While he was tying the units together, three of the metal shelves fell and pinned Employee #1 beneath them with his head and chest resting against a smaller pile of racks on the ground. Two coworkers found Employee #1 and contacted emergency services. Employee #1 was pronounced dead at the storage yard by paramedics.
