Investigation Summary
At approximately 10:30 a.m. on March 30, 2005, Employee #1, a contractor, had been working under a contract with a developer, Hamman Construction. He was working on an air conditioning unit from the elevated cage of a JLG scissors lift, rented from CAT/Hawthorne equipment. The lift was on a slight incline near the entrance of a warehouse commercial building that was being remodeled. The unit tipped over and Employee #1 fell approximately 15 to 18 feet. Various tools and some materials were in the cage at the time. Employee #1 was hospitalized in the Sharp Memorial Hospital for treatment the fracture of several bones and a head injury.
