Employee injured in fall from scaffold

On October 31, 1996, Employee #1 was working for a manpower services contractor as part of a five-man crew installing scaffold decking around the MEA tower inside the Exxon refinery in Benicia, CA. At 3:15 a.m., following a work break, he climbed the scaffold ladder toward the intermediate deck. While he was trying to step onto the deck from the ladder, his left foot caught on the toeboard under the swing gate and he tripped. Employee #1 He lost his balance and fell backward approximately 20 ft onto the concrete surface.
