Employee injured in fall from forklift at loading dock

Employee #1 was using a Hyster forklift, model S50XL , serial #C187V07516P, to load a trailer. At the same time, a truck driver backed an empty truck into the employer's northern loading dock so that he could depart in another truck being loaded at the southern dock. The driver got into the truck being loaded and pulled it forward a few feet just as Employee #1 was backing out of the trailer. The forklift became caught between the trailer and the dock, and Employee #1 jumped to the ground. He suffered lacerations and nerve damage to his lower right arm and hand. The driver had not followed established dock procedures, including checking the trailer to ensure loading was complete, and lifting the dock plate before unchocking the tires and pulling forward a few feet to close the trailer.
