Investigation Summary

Investigation Nr: 170869036
Event: 10/13/1999
Employee injured when pinned by leaning side wall

At approximately 11:00 a.m. on October 13, 1999, Employee #1, of the Consolidated Fabricators Corporation, was welding the floor seams of preassembled panels for a large debris box. A 1,000 lb side wall panel had been brought in by a crane and placed in position. It was then welded along the bottom edge while the crane held up the wall. As Employee #1 continued welding, a coworker detached the crane and rigging and moved the crane down the assembly line. The side wall slowing began to lean over, pinning Employee #1 to the floor and fracturing his tail bone. Employee #1 was administered first aid at the scene before being transported by ambulance to UC-Davis Medical Center in Sacramento, CA. The inside bracing used to plumb the side wall was not installed after the wall was brought in by the crane.
