Employee's hand crushed in rolls of grinding mill

At approximately 12:30 p.m. on November 19, 1998, Employee #1, a grain mill operator for Pacific Grain Products, was making flour on a rice mill. As he worked, the machine began vibrating because of a buildup of rice flakes on the mill rolls. Employee #1 opened the upper door, intending to use a spatula to scrape the rolls clean when he noticed that the grate guard covering the rolls had been displaced from the pins upon which it rested. He was reaching down to push the guard back into the position when the grate turned and his right hand slipped into the rolls. He sustained crushing injuries and much of the skin was torn off.
