Investigation Summary

Employee #1, a carpenter, was standing on a 16 ft extension ladder equipped with safety feet, bolting a 2 by 6 board into a red iron post. The ladder slipped and he fell onto the concrete floor. Employee #1 broke his right elbow and three fingers on his left hand. There was dirt and sawdust on the floor under where Employee #1 had placed the ladder.
