Investigation Summary

Investigation Nr: 201102795
Event: 05/14/2001
Employee's thumb amptutated

The employer had removed brushes from a bottle conveyor to make the movement of bottles more efficient. The brushes had been pushing the bottles at the neck faster than the conveyor was moving. This conveyor was below the operator's view, and the removal of the brushes exposed a small portion of the sprockets for the brushes' drive chain. Since the bottles were not being pushed together as they once were when the brushes were in place, the operator, Employee #1, had to manually push the bottle closer together. When doing so, Employee #1 placed the thumb of her right hand up into the wheel and sprocket of the conveyor, amputating the tip of her thumb.
