Investigation Summary
At approximately 4:30 a.m. on December 19, 2001, Employee #1 was using an elevated forklift to place a pallet on a tarp-covered stack of almond bins approximately 16-ft high. He left the platform of the forklift, walked along the rim of the top bin, slipped, and fell to the ground. Employee #1 was hospitalized for a fracture.
