Employee injured while operating forklift

At approximately 6:50 a.m. on February 11, 2002, Employee #1 was operating a stand-up forklift. While operating in reverse, the employee attempted to turn the forklift into an aisle. She lost control and caught her left leg between the forklift and a metal rack. The employee sustained multiple lacerations and a fracture to her lower left leg and she was hospitalized
