Investigation Summary
An employee was attempting to remove a jam in the fruit cup filling machine by using water to release the frozen product that may have been stuck inside the cylinders. Employee #1, the foreman, came to see why the production was still stopped. Without checking to see if the coworker had properly locked out and deenergized the machine, Employee #1 stuck his finger in the filler tube and the machine activated. Employee #1 suffered the amputation of his finger at the second joint and was hospitalized.
