Employee contacts blade and has thumb amputated

Employee #1, a 32-year-old carpenter was assigned to a table saw to cut wood pieces in 3.125-in. strips. Employee #1's only experience with a table saw was in high school. Though instructed on how to perform the task, Employee #1 was not informed on the requirements of a hood requirement to guard the blade above the table and material. As Employee #1 pushed the wood forward with the left hand towards the unguarded saw, he bent down to the right to grab the next piece of wood leaning against the saw table. Employee #1 reached down with the right hand, dipping the shoulder, and his left hand turned and his left thumb came into contact with the rotating blade amputating the thumb above the knuckle. Employee #1 knew what a push stick was, but did not use it on this machine. He was hospitalized for his injury.
