Investigation Summary
On October 29, 2001, a section of a conveyor belt was out of service. A metal tray was placed on top of the broken section so that the product could be manually slid to the next running conveyor belt. Employee #1 grasped the ends of the metal tray to remove it. His left ring finger was smashed between the end of the metal tray and the permanent rib on the conveyor belt. He was treated a hospital and released with an amputated finger tip.
