Emoloyee Injured In Fall From Fixed Platform

Employee #1 was working on an elevated metal platform (called a "Beach") inside a DAF (a DAF is similar in configuration to a clarifier at a sewage treatment plant) and was using an organic based solvent to clean brackets on the metal arm extending from the center of the DAF. The "Beach" was not equipped with a guard rail, and a body harness and lanyard system was not in use even thought the injured worker was exposed to a 7.5 ft fall hazard. Employee #1 left his work area by walking along the unprotected edge of the "Beach" to the outside of the DAF. Employee #1 attempted to grab a fixed guard rail attached to the outside of the DAF, however, his attempt failed. Employee #1 fell back onto the "Beach" and then rolled off the "Beach" falling approximately 7.5 ft to the bottom of the DAF. Employee #1 was hospitalized.
