Employee Is Killed in Fall from Roof

Employee #1, a laborer of a roofing material supplier, was loading material onto the roof of a two story house. Employee #1 fell from the second-story roof onto the roof of the first story, and then fell 9.25 feet to a concrete pad. Employee #1 was not wearing fall protection. Employee died of head injury.
