Investigation Summary

Employee #1 was being trained to operate a manhole pipe machine at a precast concrete plant. At the time of the accident they were making 48 in. diameter concrete manhole pipes, 4 ft long with 4 in. thick walls, with each section weighing 3,000 lb. The press equipment was automated, with pipe sections rotating on a large dial feed; the operator's station was well away from the actual forming process. For some reason, Employee #1 put his left hand on the press head. When the head raised automatically as part of its cycle, his hand became caught between the head and the form jacket, amputating four of his fingers.
