Employee's finger injured in bag-making machine

At approximately 7:00 p.m. on January 4, 1999, Employee #1 was operating bag-making machine #6 when he noticed a buildup of residue on the draw rollers from the plastic sheeting passing between them that was causing the rolls to slip. The sheeting then tore apart, stopping production. Employee #1 took a rag in his left hand and began wiping off the residue, while resting his right hand on the machine frame, near the ON-OFF switch. The machine suddenly started, catching his finger and drawing it into the rolls. Proper lockout procedures had not been followed. Employee #1 was a machine operator for a manufacturer of plastic bags used for bulk packaging of dairy products, such as milk and yogurt.
