Employee Is Killed in Fall from Forklift

Employee #1 worked for a furniture and appliance company as a warehouse person. Employee #1 was using a forklift truck to retrieve a piece of furniture. The company refers to this forklift truck as a "picker". Employee #1 elevated himself to a height of 32 ft above the concrete floor on bin Number 19. Employee #1 fell to the floor. Employee #1 was wearing a body harness. It appears Employee #1 failed to fasten his body harness to the lanyard on the picker. Employee #1 was killed.
