Investigation Summary
On April 5, 2005, Employee #1, working for Contractors Wardrobe, was operating a vinyl application machine in which, at the point of operation, two of the rollers had a gap set by the operator of three millimeters. Above the rollers, the vinyl was applied to the glass, cut with an automated knife to size, and sent through the rollers, where the vinyl was pressed onto the glass via a conveyor. Some of the vinyl had become stuck on the rollers and was causing process problems, so Employee #1 was attempting to remove this vinyl from the rollers while the rollers were still in operation. His right hand became caught and was pulled into the rollers of the vinyl applicator. He was hospitalized for his injuries over twenty four hours.
