Investigation Summary
On June 27, 2009, Employee #1 and coworkers were plastering the exterior of the building. Employee #1 was working from the work platform of a metal scaffold, when he fell to the ground for an unknown reason. The height of the fall was approximately 12 ft. He landed upon the ground, striking headfirst. Employee #1 was transported to a medical center with severe head trauma and fractures to his neck, where he died two days later.
