Worker Crushes Thumb While Moving Metal Plate

One June 9, 2009, Employee #1, a maintenance and service manager with Beneficial Insectary Inc., was constructing a metal box. He was guiding a heavy metal plate into place when he lost control of the plate, crushing his right thumb. Employee #1's thumb was later amputated by a medical doctor.
