Investigation Summary
On June 4, 2011, Employee #1 was working as a laborer for his employer, a mushroom grower. Employee #1 was tending a belt system that emptied mushroom compost onto a conveyor that loaded the compost onto a dump truck. For some reason, Employee #1 was caught in the conveyor system, and his right arm was severed near his shoulder. Employee #1 was hospitalized.
